What this page covers

This notice covers the website only. It explains what happens to information you type into a form here. It is not the practice’s HIPAA Notice of Privacy Practices, which governs your medical record and is provided to you when you join.

What we collect

Only what you enter into the meet-and-greet form: your name, email address, phone number, what brings you to us, when it suits you to be reached, and how you heard about the practice. We also record the page you submitted from and the site that referred you, so we know which parts of the site are useful.

Do not use this website to send clinical information. The form is for arranging a first conversation, not for describing symptoms or medical history.

What we do with it

We use it to contact you about your enquiry and to arrange your visit. Your submission is stored in the practice’s systems and in the practice management platform operated by Macbach Healthcare Solutions on the practice’s behalf. We do not sell your information, and we do not share it for advertising.

Analytics

We measure how the site is used in aggregate, such as which pages are read and where visitors arrive from. No information you type into a form is ever sent to an analytics provider.

Keeping it secure

Form submissions are transmitted over an encrypted connection and are protected by a bot-verification check. Access is limited to the practice and to the staff operating its systems.

Asking us to remove it

You can ask us to delete an enquiry at any time. Call (727) 607-0504 or email admin@mychoiceprimarycare.com and we will remove it.
